Breast Reduction Techniques

Informed Consent for Breast Reduction

Through the years there have been many techniques in breast reduction that have been proposed as the ideal surgical procedure. All have distinct advantages and disadvantages which a surgeon takes into consideration in making his selection. All can yield excellent results when properly executed. Usually your physician will have a preference as to which one gives the best results in his/her hands. From the viewpoint of the patient, the scar is the most obvious difference. All techniques utilize a scar around the areola and extending down to the inframammary fold of the breast. These are the only scars normally seen in a vertical scar reduction mammaplasty. The more traditional Wise pattern or inverted T reduction mammaplasty has an additional scar transversely in the inframammary fold. Vertical scar techniques are generally quicker but don't shape up as well initially and are difficult to apply to the very large breast. Inverted T incisions yield excellent initial and final shapes, are easily adaptable to both large and small breast reductions, but take slightly longer to perform. Dr. Flory's preference is for the improved flexibility and shape offered by the traditional inverted T, inferior pedicle reduction.

This technique also allows the surgeon to remove the excess tissue on the sides, an area that is frequently ignored by the quicker techniques which are becoming more popular. Unfortunately, in an era of decreasing reimbursements, many physicians are opting for a procedure requiring less time with less regards to the quality of the results obtained.
