Breast Reduction
Breast reduction (reduction mammaplasty) is a subset of mastopexy with tissue reduction that is performed primarily to relieve symptoms related to the weight and pull of the large breast. Those symptoms include among others:

- back, neck and shoulder pain
- frequent rashes in the inframammary fold
- frequent headaches
- shoulder indentations from bra straps
- pain in the breasts with vigorous activities
- difficulty breathing when lying supine
When symptoms are severe enough and adequate medical management has failed to alleviate the problem, insurance may consider breast reduction surgery to be medically indicated and thereby provide reimbursement for the procedure. All insurance companies require that minimum amount of breast tissue be removed which is directly related to the height and weight of the patient. Additionally, recommendations for surgery from other physicians may be required. Finally, some insurance companies may insist on documentation of failed medical management from your primary care physician. Even within the same insurance company, different policies have different requirements.

Unfortunately, obtaining insurance coverage for breast reduction has become very difficult with most insurance companies. Patients desiring insurance coverage for their breast reduction surgery must have a consultation during which the amount of tissue to be removed will be determined. A predetermination letter is then submitted to their insurance company detailing the need for surgery and the amount of tissue needing to be removed. Letters from other physicians or other therapists should be submitted with the predetermination letter. Those letters will collaborate the need for surgical intervention and increase the likelihood of an approval for breast reduction surgery. The entire approval process can take from several weeks to several months or longer if appeals are made on initial rejections. Because of the uncertainty involved with insurance covered breast reductions, procedures can not be scheduled until a written approval from the insurance company is obtained.

In many cases, the individual requesting breast reduction simply cannot meet the requirements set by their insurance company. Breast reduction can still be performed on a self-pay basis.
